Aspirin is a NSAID drug that is absorbed primarily in the small intestines. This type of drug can affect our kidneys and cause acute, reversible impairment. This occurs because aspirin inhibits COX-1, thereby depriving the kidney of prostaglandins needed for normal function.  If a patient experiences a decrease in urine output or a rise in serum creatinine/BUN levels, the patient should discontinue the drug. With that being said, most patients kidney function return back to normal.

It follows that this medication should be used with caution in patients with renal impairment and the dose adjusted accordingly. It is also important to monitor the interactions with other drugs because use of two potentially nephrotoxic drugs together may cause greater risk of nephrotoxicity. Assessment should include evaluation of the 8th cranial nerve function and monitoring of the serum vancomycin levels of people with renal impairment and/or patients over the age of 60. It is also important to monitor the ratio of intake and output. If there is any sign of cloudy or pink urine, it may a detection of nephrotoxicity. Naproxen, also known better as Aleve, is an over the counter NSAID. It is used most commonly for disorders of mild to moderate pain, such as rheumatoid arthritis and osteoarthritis. Since several chronic health conditions require continued use of Naproxen as an analgesic, its continued use could cause severe kidney damage.

Naproxen can affect the kidneys by causing acute renal failure, as well as cystitis and hematuria.

Naproxen is absorbed from the GI tract, and excreted through the urine.

Nursing considerations: we need to keep in mind other drugs, such as antibiotics or other pain killers, that are nephrotoxic as well while administering Naproxen. In renal patients, we must be extremely cautious of its use, due to the kidneys extra time needed to process the drug. Naproxen can also cause an increase BUN, serum creatinine, and serum K due in decreased kidney function. Adequate hydration is recommended for patients taking Naproxen with impaired renal function.
